The questions on this application are for our use in determining if you are a suitable candidate with which to place one of our avians. It is also used to determine if the bird you wish to adopt is suitable for your environment.

I understand there will be a nominal adoption fee that is to be paid before the bird is placed in my home, to help offset the costs Casey's Nest Foundation has incurred saving this bird.

I understand that Casey's Nest Foundation holds no responsibility for the health, or behaviour of any of avian they offer for adoption. I understand that a bird is a living animal with feelings and temperament and they can inflict injury.

I agree to have my adopted bird checked with an avian vet within 30 days of adoption. (sooner if it shows signs of illness)

I understand that the birds offered for adoption by Casey's Nest Foundation are rescue birds and Casey's Nest Foundation will not be held responsible for them upon adoption.

I agree to treat my adopted bird with consideration, love, and respect.
